MAR'S

Medication Administration Records for recording the dispensing of meds in a long term care facility. Some are combination physicians order forms attached to MAR's and others are individual forms. Although developed for specific software systems most have been used for proprietary or other software providers.

Physician's Order Form #34109

Continuous three part pin fed form, 9-3/4" x 11" overall, detached size 8-7/8" x 11", all parts are 3 hole punched on left edge, part one (chart copy) is white carbonless paper, part two (pharmacy copy) is canary carbonless paper, part three (charting copy) is 28# white ledger, between parts two & three is a pattern carbon page, all parts print reflex blue and yellow on front, the back of part three prints gray. Each carton contains 1100 forms.
